
Home Medicines Review
A Home Medicines Review (HMR) provides a thorough check of a patient's medicines by a Pharmacist at the request of the patient's GP and with their agreement. The HMR ideally takes place in the patient's home. Useful resources for GPs, Pharmacists and Practice staff can be found at the bottom of this page.
Patients who could benefit from a HMR are those:
who take more than five medicines a day
who are confused or worried about their medicines
who sometimes forget to take their medicines
who see more than one GP or specialist
who have recently spent time in hospital.
How does a HMR help?
A Pharmacist will visit patients in their own home and
show patients how to take their medicines correctly
explain why and when medicines should be taken
explain where medicines should be stored
check that the prescription medicines, over the counter medicines and vitamins are appropriate to take together
check that equipment is in working order and
check that patients are using the correct technique.
