As a result of generous funding from ALCOA, GP down south has been providing the services of a Case Manager at Peel Youth Medical Service (PYMS) since 2016. As the Case Manager, Joseph Gauci has been providing young people with support to access a range of community, clinical and social networks in an effective, efficient and timely manner. Unfortunately, the funding for the Case Manager position ceased on the 30 June 2018 and we have not been successful in finding alternative funding. As a result, GP down south is no longer able to continue to provide this service. Joseph has been working closely with his case load of young people to ensure they are offered alternative and ongoing support as needed, including utilising current services within PYMS. GP down south would like to thank ALCOA for their support over the past two years. We also thank Joseph for his dedication and commitment to improving the health and wellbeing of local youth. Joseph will continue providing support to the local community through his role as Care Coordinator in the Assertive Outreach Program.
