Collaborative Patient Centred Care: A Pathway for Improved Health Outcomes

Our Chronic Conditions Program has recently been providing a range of education workshops for GPs, Practice Nurses, Pharmacists and other Allied Health Professionals.
The Chronic Condition Care Coordination Service, GP Lead Dr Russell Wallis led the way in developing a three-part webinar series covering My Health Record.  These were attended by over 40 participants.  Topics covered Improving Outcomes for Patients with Chronic Conditions, Action Plans, Patient Education and Home Medicine Reviews and Chronic Conditions Management.
Further face to face workshops were delivered to GPs and Pharmacists in Bunbury and Busselton around the role of My Health Record in improving management of chronic conditions and what role GPs and Pharmacists play.  Dr Russell Wallis and Community Pharmacist, David Williams presented on the role of My Health Record in promoting integration between pharmacists and GPs facilitating improved patient care and health outcomes, the role of pharmacy in the management of people with chronic conditions, including the benefits of Home Medicines Reviews.  Simon Benge the My Health Record Team Leader from the WA Primary Health Alliance also presented on both evenings providing the audience current information on the uptake and utilisation here in the South West  and additional clinical information now available via the My Health Record.
