Integrated Chronic Disease Care Team Services Expansion

Two new teams members have been welcomed to the Integrated Chronic Disease Care  (ICDC) team recently.  The team currently includes credentailled diabetes educators & care coordinators with a  focus on improving the integration and coordination of services for people with chronic conditions such as chronic diabetes, cardiac (CHF) or respiratory conditions (including asthma and Chronic Obstructive Pulmonary Disease COPD).

We welcome a qualified dietician, Shannen who will provide you with high quality clinical dietetic services including assessment and interventions with targeted chronic disease (COPD, asthma, diabetes and cardiac conditions). 
 
Food plays a key role in our physical and mental health and wellbeing. We all have different health challenges so there is no ‘one size fits all’ approach when it comes to food and nutrition. Dietitians understand how nutrition impacts the body and use this knowledge to treat a range of medical conditions to improve wellbeing through lifestyle and diet changes.
 
Olivia joins the team as an exercise physiologist.  Olivia's role will be to help you know how to apply exercise as a means to improve the symptoms and underlying cause. 

As part of the team, she will provide high quality assessment and develop individualised physical activity programs for clients with targeted chronic disease (Chronic Obstructive Pulmonary Disease COPD, asthma, diabetes and cardiac conditions).
 
Both Oliva and Shannen will be based in the Warren-Blackwood and will work with the rest of the ICDC team to provide integrated care.  To find out more please contact us on 9754 3662.
