Our Chronic Conditions program keeps growing and delivering results to the south west community. GP down south is delivering professional development opportunities to medical professionals and pharmacists on “Managing Chronic Conditions in South West WA. The education session will cover improving health outcomes for people with chronic conditions and managing chronic obstructive pulmonary disorder (COPD) to maximise function and prevent hospitalisation. The aim of the presentation is to update participants’ knowledge regarding best practice guidelines in the management of COPD, including the benefits of utilising care plans / action plans and the home oxygen prescription guideline. Health professionals are still coming to terms with the prevalence of chronic conditions and the implications for the health system and the economy. A presentation from our funders, WA Primary Health Alliance, will give participants an understanding of state and local context along with the strategic direction, priority areas, integration & collaboration and increasing access to primary care and pharmacy services for patients suffering with chronic conditions. There is now a Chronic Conditions Care Co-ordination Team, based at the Bunbury Regional Hospital including a pharmacist, Hospital to Home Clinical Care Co-ordinator and a soon to be GP Lead position. This multidisciplinary team will ensure smooth transition from the hospital to the home and provide support to patients to minimise hospital admissions and stays. An opportunity exists for a GP to join the team in the GP Lead position and we will be advertising shortly. If you are interested in working as a member of the Chronic Conditions Care Coordination team to provide patient focused medical care/advice that meets best practice guidelines and supports self-management in the primary care setting through the development and implementation of chronic disease care/action plans in consultation with the patient’s hospital and primary care teams, please contact Nicola Hilyard or Amanda Poller on 9752 3662 for more information.
